Patient Communications
People-Centered Health communicates with patients through several channels including in-person, by phone, by email, and by SMS text message. Mobile phone numbers and consent to receive text messages are collected during patient registration through a written Patient Communication Consent Form. Phone numbers and SMS opt-in information are not collected through our website.
Mobile phone numbers and SMS opt-in information are used solely for communication between People-Centered Health and the patient. No mobile information is shared with third parties or affiliates for marketing or promotional purposes. Information sharing with subcontractors that support our text messaging service (such as our messaging platform provider) is permitted only as strictly necessary to deliver SMS messages, and is governed by data protection agreements with those subcontractors.

How We Share Information
